These are provided for your convenience and will save time when you arrive for your scheduled visit. Please print the appropriate files provided below, fill these out prior to your next appointment, then hand these to a front desk staff member upon your arrival. Thank you for being a valued patient!
New-Patient Forms
Welcome aboard! It is important that we have detailed information about you to allow us to provide dental care for you in a safe and efficient manner. For these reasons, it is necessary that paperwork be completed prior to treating you as a patient.
However, for your convenience we automatically send out all of your new patient forms via email and sms text shortly after your new patient appointment has been scheduled. If you have any questions while completing your new patient forms, please feel free to contact our office and a team member will be happy to assist you.
1) Complete the "New Patient Online Questionnaire" provided below online then submit this electronically using secure HIPAA-compliant encryption; or
2) Print the appropriate pdf. files provided below, fill these out prior to your first appointment, then hand these in upon your arrival.
